RESUME
Au Bénin, la prise en charge de la malnutrition aiguë sévère
fait partie des priorités des autorités sanitaires et gouvernementales au plus
haut niveau. Cet engagement a trouvé un écho favorable auprès des partenaires
techniques et financiers tel que l'UNICEF qui accompagne depuis 2007 le
gouvernement béninois dans l'élaboration et la mise en œuvre du protocole
national de prise en charge de la malnutrition aiguë. Un audit clinique a été commandité par les
responsables sanitaires des départements du Borgou et de l'Alibori pour
apprécier le niveau de mise en œuvre de ce protocole au centre nutritionnel
thérapeutique de l'Hôpital Évangélique de Bembèrèkè.
Cet audit qui a porté sur 34 enfants de 6 à 59 mois admis au
centre nutritionnel thérapeutique de l'Hôpital Évangélique de Bembèrèkè au
cours du second semestre de l'année 2010. Elle a permis de mettre en évidence
le non respect du protocole par les prestataires du centre nutritionnel. En
effet, il est apparu que même si tous les enfants admis font l'objet d'un
examen clinique complet, le test d'appétit n'est réalisé chez aucun d'eux et le
suivi au cours de l'hospitalisation ne va pas à son terme. L'équipe projet est
parvenue à la conclusion qu'il est indispensable d'adopter la pratique
régulière de l'audit clinique pour améliorer la prise en charge des enfants de
6 à 59 mois malnutris aigus sévère.

Abstract
In Benin, the
management of severe acute malnutrition is a priority of health authorities and
government at the highest level. This commitment has found support from
technical and financial partners such as UNICEF, which supports since 2007 the
Benin government in the development and implementation of the national protocol
for management of acute malnutrition. A clinical audit was commissioned by
health officials of the departments of Borgou and Alibori to assess the level
of implementation of this protocol in therapeutic feeding center of the
Evangelical Hospital Bembèrèkè.
This audit covered a sample of 34 children aged 6 to 59 months admitted to therapeutic feeding center of the Evangelical Hospital Bembèrèkè in the second half of 2010. It helped to highlight the non-compliance by providers in the feeding center. Indeed, it appeared that even if all the children admitted are subject to a full clinical examination, the test performed in appetite is not one of them and monitoring during hospitalization will not to an end. The project team came to the conclusion that it is essential to adopt the regular practice of clinical audit to improve care for children from 6 to 59 months severe acute malnutrition.
